Creating a form

Create a Form by opening Forms, clicking new, naming it, picking the lines of business it covers, and configuring the steps. SALT auto-generates a primary Link so the Form is shareable as soon as it's saved.

What you’re building

A Form is the reusable template for a particular kind of intake — auto, home, renters, commercial, a referral partner flow, a renewal capture. The Form defines the questions, the steps, the integrations that fire, and the look. To share the Form with prospects, you use a Link — a token-based URL that points at the Form.

You don’t share the Form directly. You share Links to it. One Form can have many Links, each branded differently or assigned to different team members. See Creating form links for the Link side.

Create a Form

    Open the Forms section from your agency’s main nav.

    Click the new-Form action (commonly labeled New form or similar).

    Give the Form a clear name. The name shows up internally in your account and (depending on settings) on the consumer’s screen.

    Pick the lines of business this Form covers. The choice determines which standard SALT questions are available.

    Save. SALT creates the Form and an initial primary Link in the same step.

The new Form has a default structure based on the lines of business you picked. From there, customize.

After it’s created

Most agencies take these next steps before sharing the Form:

You don’t have to do all of these immediately — you can ship a Form with the defaults and refine over time.

Using a Form once it exists

Once the Form has at least one active Link, you can:

  • Send the Link to prospects (email, SMS, social media, embed on your website).
  • Pull up the Link’s QR code for print or in-person sharing.
  • Add additional Links for different campaigns or referral partners — see Creating form links.
  • Use the Form as a Live Intake target — start a Live Intake session on a Submission and the Form’s structure drives the questions.

Common questions

Do I need to “publish” a Form? There’s no separate publish step. As soon as a Form has an active Link, that Link’s URL works for consumers.

Can I have a draft Form that’s not shareable yet? Archive the auto-generated Link if you want the Form ready but not yet live. Re-enable when you’re ready to share.

What’s the difference between a Form and a Live Intake template? A Form is what consumers fill out (or what an agent walks through during a Live Intake call). A Live Intake template is a separate structure that organizes the agent’s call experience. They can pull from the same question library but are different objects.

Can I copy questions from one Form to another? The fastest path is to duplicate the Form and then edit. Cross-Form question copying isn’t a separate operation.
