What this is
Your insurance agency uses SALT to collect the information they need to provide you with a quote or process your request. The form on your screen asks for the details they need — vehicles you want insured, property you own, drivers in your household, and so on.
What to expect
- Multiple steps — the form is broken into sections. You’ll move through them one at a time.
- A mix of question types — typing, dropdowns, multi-select boxes, occasionally document uploads.
- Required fields — some questions must be answered to advance. Optional questions are clearly marked.
- Progress indicator — most forms show how far through you are.
Total time depends on the form. Quick quotes can be 5-10 minutes; comprehensive intakes might be 15-30.
Saving and pausing
Your answers save automatically as you click through. If you close the browser tab, lose your connection, or run out of time:
- If you’re partway through, your agency may email you a link to come back to where you left off
- If you completed it, you should see a confirmation page (and possibly an email)
If you don’t get a resumption email and want to come back, contact your agency directly — they can send you a continue link.
Tips for filling it out
- Have your existing policy info handy if you’re switching from another carrier — declarations page, current policy number, current carrier name
- Have driver details for everyone in your household if it’s an auto form — license numbers, dates of birth, marital status
- Have property details if it’s a home form — square footage, year built, recent renovations
- Be honest — wrong information leads to wrong quotes; the eventual policy will reflect what you stated
What happens after you submit
Once you complete the form:
- Your information is delivered to your agency
- They reach out to discuss next steps — usually within a business day
- You may receive a confirmation email letting you know they got it
Phone verification
Many forms ask you to verify your phone number via a text message code. If you don’t receive the code within a minute, request a resend. Some carriers occasionally delay SMS — try again or use email contact instead.
Document uploads
Some forms ask you to upload documents — your current declarations page, a photo of your driver’s license, etc. The form accepts standard formats (PDF, JPEG, PNG, GIF, WEBP) up to 25 MB per file.
On mobile, you can take a photo directly with your camera — usually faster than uploading from your gallery.
Common questions
Is my information secure? SALT is built for insurance data with appropriate security measures. Sensitive fields are protected in storage and only the agency you’re working with (and their authorized staff) can see your responses. If you have questions, ask the agency directly.
Will I get spam from filling this out? The form includes consent terms — typically you’re agreeing to be contacted about your insurance request, not for unrelated marketing. Read the consent text on the form for specifics.
Can I edit my answers after submitting? Once submitted, edits go through your agency. Contact them to update anything that needs to change.
The form crashed mid-way. Did I lose my answers? Your agency can usually pick up the partial submission and send you a continue link to finish. Reach out to them.
